Cardiocoach is a service focused on prevention and education in healthy habits, aimed at people who want to take control of their cardiovascular health.
This service is available both for those with low cardiovascular risk or without a confirmed diagnosis (“primary prevention”), and for those with confirmed cardiovascular risk or who have suffered a cardiac event (“secondary prevention”).
The service is provided by a multidisciplinary team made up of a cardiologist, a family doctor, and a nutritionist to give you the best support. Get the care you need from the comfort of your home, at work, or while traveling.
Once the service is activated, the medical team responsible for the program will contact you to gather information and perform an initial assessment.
Your cardiovascular risk assessment will be conducted through your medical history and a questionnaire to assign you the most suitable program.
Based on the results obtained, you will be assigned to a Primary Prevention program (low risk) or Secondary Prevention program (confirmed risk or history of cardiac events), as appropriate.
You will have ongoing follow-up from your medical team and nutritionist, who will provide you with the most appropriate tools, recommendations, and health education tailored to you.
At the end of the follow-ups, a new assessment will be carried out to adjust the plan to your specific needs, so that you can be autonomous in your own care, or you may opt for a semi-annual follow-up to ensure better control.
